The compound was developed in the 1980s at the Institute of Molecular Genetics in Moscow, where it emerged from research on short ACTH fragments and their effects on the central nervous system. Russian pharmaceutical listings describe it as a nootropic and neuroprotective agent, most often formulated as nasal drops. It is not a marketed medicine in the United States or the European Union, and no pharmacopoeial monograph covers it. Consequently, most published clinical experience with the substance originates from a small number of research centres, mainly in Russia and neighbouring countries.
Pharmacological accounts link semax to melanocortin signalling and to modulation of neurotrophic factor expression, particularly brain-derived neurotrophic factor and nerve growth factor. Much of this evidence comes from rodent studies using intranasal delivery, a route chosen because it allows peptides to reach the central nervous system with limited systemic exposure. Whether the same mechanisms operate in humans at comparable magnitude remains an open question. The precise receptor or receptors responsible for the reported behavioural and neuroprotective effects have not been conclusively identified.
Clinical reports describe use in ischaemic stroke, transient ischaemic attack, optic nerve conditions, and cognitive complaints, but most of these studies are small and were conducted in a single region. Systematic reviews have generally described the evidence base as limited in size and variable in methodological quality. Randomised controlled data suitable for international regulatory assessment are scarce. As a result, major treatment guidelines outside Russia do not include the peptide, and interest in it remains largely research-driven rather than routine clinical.

The International Union of Pure and Applied Chemistry (IUPAC) defines "coenzyme" a little differently, namely as a low-molecular-weight, non-protein organic compound that is loosely attached, participating in enzymatic reactions as a dissociable carrier of chemical groups or electrons; a prosthetic group is defined as a tightly bound, nonpolypeptide unit in a protein that is regenerated in each enzymatic turnover. Some enzymes or enzyme complexes require several cofactors. For example, the multienzyme complex pyruvate dehydrogenase at the junction of glycolysis and the citric acid cycle requires five organic cofactors and one metal ion: loosely bound thiamine pyrophosphate (TPP), covalently bound lipoamide and flavin adenine dinucleotide (FAD), cosubstrates nicotinamide adenine dinucleotide (NAD+) and coenzyme A (CoA), and a metal ion (Mg2+). Organic cofactors are often vitamins or made from vitamins. Many contain the nucleotide adenosine monophosphate (AMP) as part of their structures, such as ATP, coenzyme A, FAD, and NAD+. This common structure may reflect a common evolutionary origin as part of ribozymes in an ancient RNA world. It has been suggested that the AMP part of the molecule can be considered to be a kind of "handle" by which the enzyme can "grasp" the coenzyme to switch it between different catalytic centers.
=== Routes of exposure === Antineoplastic drugs are usually given through intravenous, intramuscular, intrathecal, or subcutaneous administration. In most cases, before the medication is administered to the patient, it needs to be prepared and handled by several workers. Any worker who is involved in handling, preparing, or administering the drugs, or with cleaning objects that have come into contact with antineoplastic drugs, is potentially exposed to hazardous drugs. Health care workers are exposed to drugs in different circumstances, such as when pharmacists and pharmacy technicians prepare and handle antineoplastic drugs and when nurses and physicians administer the drugs to patients. Additionally, those who are responsible for disposing antineoplastic drugs in health care facilities are also at risk of exposure. Dermal exposure is thought to be the main route of exposure due to the fact that significant amounts of the antineoplastic agents have been found in the gloves worn by healthcare workers who prepare, handle, and administer the agents. Another noteworthy route of exposure is inhalation of the drugs' vapors. Multiple studies have investigated inhalation as a route of exposure, and although air sampling has not shown any dangerous levels, it is still a potential route of exposure. Ingestion by hand to mouth is a route of exposure that is less likely compared to others because of the enforced hygienic standard in the health institutions. However, it is still a potential route, especially in the workplace, outside of a health institute.
== Principles == Electron-capture dissociation typically involves a multiply protonated molecule M interacting with a free electron to form an odd-electron ion. Liberation of the electric potential energy results in fragmentation of the product ion.

=== Solvent choice === An eluotropic series, which orders solvents by how much they move compounds, can help select a mobile phase. Solvents are divided into solvent selectivity groups. Using solvents with different elution strengths or different selectivity groups can often give very different results. While single-solvent mobile phases can sometimes give good separation, some cases may require solvent mixtures. In normal-phase TLC, the most common solvent mixtures include ethyl acetate/hexanes (EtOAc/Hex) for less-polar compounds and methanol/dichloromethane (MeOH/DCM) for more polar compounds. Different solvent mixtures and solvent ratios can help give better separation. In reverse-phase TLC, solvent mixtures are typically water with a less-polar solvent: Typical choices are water with tetrahydrofuran (THF), acetonitrile (ACN), or methanol.
=== The Fish Farmers clan === The Fish Farmers clan (Dutch: De Visboeren) are led by the owners of the well-known fishmonger De Antwerpse Visser in the districts of Borgerhout and Kiel in Antwerp. The organisation is led by the brothers Mohamed A., Rachid A. and Hassan A., who quickly established themselves as main contributors within the international cocaine trade. They established the organisation in the early 2010s and took control over the cocaine trade throughout Flanders. Dozens of containers from Colombian cartels were trafficked through the organisation of the Fish Farmers clan. They very quickly competed against other organisations within the Moroccan mafia. In 2018, the Kali-team (an anti-Moroccan mafia police unit from Antwerp) discovered that the Fish Farmers were amongst the main owners of a luxury suite in the Hilton hotel near the Tangier-Ville railway station in Morocco. In March 2020, the three brothers and their father Mohamed (73) were sentenced up to eight years in prison.
== Mechanism of action == Endorphins are released from the pituitary gland, typically in response to pain, and can act in both the central nervous system (CNS) and the peripheral nervous system (PNS). In the PNS, β-endorphin is the primary endorphin released from the pituitary gland. Endorphins inhibit transmission of pain signals by binding μ-receptors of peripheral nerves, which block their release of neurotransmitter substance P. The mechanism in the CNS is similar but works by blocking a different neurotransmitter: gamma-aminobutyric acid (GABA). In turn, inhibition of GABA increases the production and release of dopamine, a neurotransmitter associated with reward learning.

=== Examining what occurs at the cellular level of NL. === Tissue Architecture in NL Lesions A lesion is a tissue that has been altered or injured. They can occur as wounds or, in the case of progressive NL, ulcers. The tissue architecture of NL is characterized by degeneration of collagen in the dermis and subcutaneous layers. The lesions exhibit granulomatous inflammation, palisading granulomas, and thickened blood vessels. Palisading granulomas are significant because they show an immune cell ring around degenerated tissue, commonly seen in autoimmune and chronic inflammatory diseases. Thickened blood vessels occur due to an accumulation of immune cells. Together, all these structural features indicate NL as a chronic, inflammatory response. Fibroblasts and endothelial cells are malfunctioning, and there is an imbalance in tissue homeostasis. Fibroblast and collagen remodeling Fibroblasts contribute to the formation of connective tissue, collagen, and elastin. Failure in fibroblasts causes skin to atrophy and degenerate. Increased uptake of GLUT-1 (a glucose transporter) is observed in NL cases. When up-regulated: glycolysis, oxidative stress, and fibroblast proliferation all increase. Despite the metabolic increase, fibroblasts in NL dysfunction. This suggests that in NL tissue decay and dysfunction are linked to both structural and immunological tissue components. The tissue is observed to have increased GLUT-1 and decreased pro-collagen mRNA. A predominance of Type 1 collagen is observed in PL.
The primary cell wall derives its notable tensile strength from cellulose molecules, or long-chains of glucose residues stabilized by hydrogen bonding. Cellulose chains are observed to align in overlapping parallel arrays, with the similar polarity forming a cellulose microfibril. In plants, these cellulose microfibrils arrange themselves into layers, formally known as lamellae, and are stabilized in the cell wall by surface, long cross-linking glycan molecules. Glycan molecules increase the complexity of the potential networks plant-based cellulose can configure itself into. Coextensive in the primary cell wall to both cellulose microfibrils and complementary glycan networks, is pectin which is a polysaccharide that contains many negatively charged galacturonic acid units. Additionally, cellulose microfibrils also contribute to the shape of the plant via controlled-cell expansion. The stereoscopic arrangement of microfibrils in the cell wall create systems of turgor pressure which ultimately leads to cellular growth and expansion. Cellulose microfibrils are unique matrix macromolecules, in that they are assembled by cellulose synthase enzymes located on the extracellular surface of the plasma membrane. It is believed that the plant can "anticipate their future morphology by controlling the orientation of microfibrils" by a mechanism where cellulose microfibrils are arranged atop a cortical array of microtubules.
In normal bone, fractures occur when there is significant force applied or repetitive trauma over a long time. Fractures can also occur when a bone is weakened, such as with osteoporosis, or when there is a structural problem, such as when the bone remodels excessively (such as Paget's disease) or is the site of the growth of cancer. Common fractures include wrist fractures and hip fractures, associated with osteoporosis, vertebral fractures associated with high-energy trauma and cancer, and fractures of long-bones. Not all fractures are painful. When serious, depending on the fractures type and location, complications may include flail chest, compartment syndromes or fat embolism. Compound fractures involve the bone's penetration through the skin. Some complex fractures can be treated by the use of bone grafting procedures that replace missing bone portions. Fractures and their underlying causes can be investigated by X-rays, CT scans and MRIs. Fractures are described by their location and shape, and several classification systems exist, depending on the location of the fracture. A common long bone fracture in children is a Salter–Harris fracture. When fractures are managed, pain relief is often given, and the fractured area is often immobilised. This is to promote bone healing. In addition, surgical measures such as internal fixation may be used. Because of the immobilisation, people with fractures are often advised to undergo rehabilitation.

It is described as a synthetic analogue of the ACTH(4–10) fragment, a short segment of adrenocorticotropic hormone. Its sequence differs from that fragment and includes two proline residues, which influence stability and behaviour in solution.
It appears in Russian pharmaceutical listings as a nasal formulation, but it is not an authorised medicine in the United States or the European Union. Outside those markets it is normally encountered as a research chemical rather than a prescription product.
Laboratory and animal work points to melanocortin signalling and changes in neurotrophic factor levels, especially brain-derived neurotrophic factor. The exact receptor targets and the degree to which these findings transfer to humans are still unresolved.
